About Elevated Health MD
Elevated Health MD is a telehealth practice based in Atlanta that does one thing: help Georgia residents get and keep their Georgia Medical Cannabis Card, with physicians licensed here and a team that knows this state’s law inside out.
Our mission
Our mission is to empower individuals to elevate their health and wellness through the safe and responsible use of medical cannabis, while promoting education, research, and advocacy for its therapeutic benefits.
Georgia roots
Plenty of card services operate from out of state and treat Georgia as one market among fifty. We are the opposite: an Atlanta practice serving only Georgians, under Georgia rules.
Our office is at 3050 Amwiler Rd in Atlanta. When you call, you reach a team in Georgia that can look up your file, not an out-of-state call center.
State law requires certification by a physician fully licensed in Georgia with a principal practice location in the state. That is the only kind of physician we work with.
From the metro to the coast, any Georgia resident can complete the entire evaluation from home. Most visits take 10 to 15 minutes by phone or video.
SB 220 rewrote the program in 2026: new name, new possession limits, vaporization for patients 21 and older, and pharmacy dispensing. Our guidance tracks the law as it stands today.
Our dispensary partners across the state offer perks and discounts to our patients through the Elevated Health rewards program.
We host and attend events around Georgia and publish plain-language guidance so patients understand their rights before they ever pay us anything.
Serving patients across Georgia, including Atlanta, Chamblee, Marietta, and Stockbridge.
How we work
Sign up online, fill out the intake form, and book an appointment at a time that works for you. We review your condition before you pay for anything.
A 10 to 15 minute telehealth consultation to evaluate your condition and answer your questions about medical cannabis treatment.
After DPH verifies your certification, you pay the $33.75 state fee from their email and your card ships by UPS, typically within 10 business days of payment.
Our dedicated team
Healthcare professionals first. Every patient works with real clinical and care-coordination staff, not a form-processing pipeline.
Yasmen holds an Associate Degree in Nursing, a Bachelor’s in Healthcare Management, and a Master’s in Clinical Psychology, with over 16 years of experience across patient care, healthcare coordination, and behavioral health. She is currently pursuing a Ph.D. in Psychology.
Her approach is holistic and patient-centered: advocacy, care planning, and guiding patients and families through complex medical and psychological concerns. As an immigrant, she brings multicultural perspective and culturally responsive care to a practice built on making patients feel heard, valued, and empowered.
Princess is a Registered Nurse with over 9 years supporting medical and behavioral health practices: patient care coordination, medical billing, insurance verification, prior authorizations, and comprehensive provider support.
She is the reason the paperwork side of this practice runs smoothly, bridging communication between patients and providers, reducing administrative friction, and keeping every application accurate and on time.
Work with a Georgia team
Start with the short intake form. Our Atlanta team reviews it, tells you honestly where you stand, and takes it from there.
Medical disclaimer. Elevated Health MD provides telehealth evaluations and electronic records processing. Certification is a medical decision made by a licensed physician based on your individual condition and history. No outcome is promised, and physicians are not required to certify any patient. Legal note. Elevated Health MD does not sell, dispense, distribute, or deliver cannabis products of any kind. Information here reflects Georgia law as of July 1, 2026, including Senate Bill 220. Marijuana remains a controlled substance under federal law; the April 2026 federal rescheduling of state-licensed medical cannabis to Schedule III does not legalize possession outside a state program.
